Rays ace Shane McClanahan will require Tommy John surgery next week, reports Marc Topkin of the Tampa Bay Times (Twitter link). He’s expected to miss the entire 2024 campaign.
It’s a devastating if not entirely unexpected development. The two-time All-Star left his start on August 2 after experiencing forearm discomfort. Tampa Bay has been exploring their options for the past couple weeks, though the club had already confirmed he was expected to miss the remainder of 2023 and that surgery was a consideration. He’s already on the 60-day injured list.
Surprising or not, it’s hard to overstate the significance of losing a pitcher of McClanahan’s caliber. The former first-round draftee debuted with 25 starts of 3.43 ERA ball two seasons ago. By his second major league season, he’d broken through as one of the game’s best pitchers. McClanahan was a legitimate Cy Young contender into August last year before a shoulder impingement cost him a couple weeks.
The absence dropped the South Florida product to sixth in Cy Young balloting, but he still finished his sophomore campaign with a 2.54 ERA through 166 1/3 frames. While his ERA took a bit of a step back this season, a 3.29 mark over 21 starts still ranks 19th among big league starters (minimum 100 innings). He’s 23rd among that same group with a 25.8% strikeout rate. McClanahan earned his second consecutive All-Star nod last month.
Tampa Bay has dealt with a brutal stretch of pitching injuries over the past few months. Top prospect Shane Baz has missed the entire year after undergoing a Tommy John procedure last September. McClanahan becomes the third member of the Rays’ season-opening starting staff to require surgery this year. Jeffrey Springs required a Tommy John procedure in April; Drew Rasmussen underwent flexor tendon surgery last month.
Even after the absences to Baz, Springs and Rasmussen, the Rays had an effective top of the rotation. McClanahan, Tyler Glasnow and Zach Eflin made for an excellent trio. With McClanahan now out of the picture, Glasnow assumes the role of staff ace. Eflin — who has dealt with knee issues throughout his career but has a 3.67 ERA through 23 starts in his first season with Tampa Bay — is the #2 option.
The early-season injuries led the Rays to acquire Aaron Civale from the Guardians at the deadline. That acquisition takes on increased importance with McClanahan out. Still, there’s little recourse beyond internal options for Tampa Bay in patching together the final two rotation spots. Taj Bradley has been up-and-down as a rookie and is on optional assignment to Triple-A. The Rays have pushed Zack Littell into the rotation and recently activated Josh Fleming from the injured list.
Tampa Bay enters play Tuesday three games back of the Orioles in the AL East. They’re still in strong playoff position, 7.5 games clear of the Mariners (the top non-playoff team in the AL). Their chances of running down Baltimore to secure a first-round bye and/or making a deep run once they get into October are obviously dealt a hit by the loss of one of the game’s best starters.
This is the second Tommy John procedure of McClanahan’s career. He missed his freshman year at USF after undergoing surgery in 2016. The Rays figure to be especially cautious with his recovery as a result, although there’s a good chance he would have missed all of ’24 even if this were his first TJS.
McClanahan has just under three years of major league service. He’ll be eligible for arbitration as a Super Two player next offseason. The Rays will need to carry him on their 40-man roster during the offseason but will immediately place him back on the 60-day IL next spring. He’ll collect service time while rehabbing and remains on track to first reach free agency after the 2027 campaign.

I hope that no minor has been abused. But I am happy when the rich and famous are fully investigated and where appropriate prosecuted for heinous crimes where ordinary working folks would be jailed. No one is above the law. But if they just posed suggestively together, while dressed, it is very possible that the investigation could show abuse or also it could show no abuse at all. Let’s hope no one was abused but most importantly, let’s get it right.

The pitch clock makes pitchers rush their deliveries which messes up their mechanics causing injury. Any time you rush anything, it will have an adverse effect.

McClanahan would have had a pitch clock his entire MiLB career berfore reaching the Majors. He should have been well used to the tempo and mechanics should not be an issue for him . If it made that big of a difference, you would be seeing a lot of long time vets blowing their arms out as the long time guys never had to deal wit h a clock. There have been a ton of TJ surgeries before the clock was instituted.

Throw the ball. Catcher throws it back. Get set. Go through your pitching motions. Throw the ball. That’s all doable in 30 seconds.
If anything it shortens recovery time between pitches. Guys throwing hard as they can or constantly throwing sliders curves that put strain on arms not able to recover as much would seem to be better bet.

I’ve got to agree – I don’t think the clock has any impact at all. If you go back (but post the original Tommy John surgery) the average game was done and dusted in about two and a quarter hours. (I’m currently enjoying watching the 1981 World Series) and more surgeries weren’t happening then.
Is it possible that the modern pitcher is a thrower rather than a pitcher?
(Caveat I’m English and a relative newbie to Baseball in 2012)

I agree. Back in the 70s when I was old enough to start paying attention to the nuances of the the game, If you had a guy that could get a FB into the 90s, he was throwing some serious heat,. The first guy that I remember hitting 100 consistently was Ryan. He was pretty much it for a lot of years. Now guys are routinely sitting in the mid to upper 90s with sliders and curves in the low 90s. Anything but a FB used to be in the low to mid 80s. Many modern pitchers are pitchers and not throwers but as you say the modern pitchers put a lot more stress and torque on their arms that are not meant to withstand that punishment. Their arms are breaking down much quicker now that they are hitting such high speeds on all of their pitches.

Tommy John. So the ligament gets replaced by a graft typically taken from one of the following tendons: Palmaris longus tendon from the forearm. Hamstring tendon. Big toe extensor tendon.
Are there stats somewhere on post surgery performance based on which tendon is grafted?
For these guys that need TJ twice does that mean the tendon graft ripped apart?
Can we double up going forward and put a combo toe/thigh or forearm/thigh graft together? Braid it up like a rope and make a super tendon?

The Rays baby their pitchers as much as any team when it comes to pitch counts and innings limits, yet their entire pitching staff is decimated with arm injuries. I’ve never understood why teams restrict resilient young early 20 year olds from being stretched out and limit them, but they’ll ride a 30+ year-old far longer and push him to high innings and pitch counts. When I was 20, I could handle a lot more physically than I can now at 34. It just doesn’t make sense to me.
Tommy John Surgery is inevitable for anyone that is throwing hard and throwing often, so why not just cut the kids loose, accept that they will likely have the surgery at some point and then get them back afterwards?
Remember when the Nats shut down Strasburg in the heat of a pennant race to preserve the health of his arm? How that’s work out?

The Baltimore Orioles rank 14th in pitching overall and have a 99.5% chance of making the postseason….once in, just like the Phillies of last season, they have as good a chance of running the table to the WS as anyone who makes the playoffs….after all the playoffs are made for players who step up when not expected to……..and all this after many years of being the leagues punchline for mediocrity……..the Red Sox and their fans can only look up at the Orioles and wonder what might have been…stick that in your beans and eat them as your typically very jealous of their success and your teams lack of……….

If the Rays can get value, I wouldn’t be shocked if they ‘diversify’ and de-risk the injured pitchers. McClanahan, Springs and Ras all coming back at the same time is going to be a lot, and Springs is now untradeable with his contract. At least 3 guys who won’t play in 2024 will take up 40-man roster spots all off-season. Could be 4 if they decide to keep Cleavinger. Trading one of McClanahan or Rasmussen makes a fair amount of sense.
